Job summary

Qualifications

• Must be graduate of an accredited nursing program and hold an LPN license in DC.
• Experience in residential or healthcare settings serving vulnerable populations preferred.
• Excellent interpersonal, written, and oral communication skills.

Responsibilities

• Consult with the person served to discuss health problems and educate on conditions.
• Develop a care plan addressing personal health care needs.
• Coordinate medical appointments and treatment plans with providers.
• Document progress and maintain Health Passport and MAR.
• Provide nursing support during on-call hours and in emergencies.
