Job summary

Heart to Heart Hospice in League City, TX seeks an LPN/LVN On-Call to support patient care in home settings under the supervision of the On Call RN. This role includes performing home visits, observing, reporting findings, and assisting with the Plan of Care and end-of-life care at patients' homes.

Requirements include graduation from an accredited school of vocational nursing, current LPN/LVN license, and at least 2 years of clinical experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse preferred.
• Two years of LVN/LPN experience in clinical care, hospice or home health preferred.
• Current State Driver's License and reliable transportation.

Responsibilities

• Under On Call RN guidance, supplement patient care per Plan of Care.
• Provide home visits to observe and report findings to the RN.
• Assist the RN in carrying out the POC and patient self-care instructions.
• Ensure compliance with policies, procedures and regulations.
• Timely submission of all documentation and related duties.
