Job summary

Heart to Heart Hospice is seeking a Licensed Vocational Nurse/Licensed Practical Nurse for PRN or full-time coverage to support compassionate care. You will assist the RN with the plan of care, educate patients and families, and document clinical progress while complying with regulations and policies.

Ideal candidates have an active TX LVN/LPN license, reliable transportation, and at least two years of clinical experience in care settings with hospice or home health preferred.

Qualifications

• Current Vocational Nurse or Practical Nurse license in Texas.
• Graduate of an accredited school of vocational nursing.
• Current driver's license and reliable transportation.
• Two years of LVN/LPN experience in a clinical care setting preferred, hospice or home health preferred.

Responsibilities

• Assist the RN in carrying out the plan of care.
• Under the direction of the RN, supplement nursing care needs as specified in the POC.
• Assist the patient in learning appropriate self-care techniques.
• Complete appropriate clinical and progress notes, documenting outcomes of interventions.
• Assess patient/family response to care and communicate with the team.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.
