Job summary

Heart to Heart Hospice is seeking an LVN/LPN on-call to support compassionate end-of-life care. Under the On Call RN/Triage RN, you will supplement patient care, perform home visits, observe and report findings, and assist with the plan of care.

You will help patients learn self-care, follow organizational policies, and ensure timely documentation. A valid LVN/LPN license, Texas driver’s license, reliable transportation and ability to work on-call shifts are required.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current State LVN/LPN license preferred.
• Two years of LVN/LPN experience in clinical care, hospice or home health preferred.
• Current State Driver's License; reliable transportation and auto liability insurance required.

Responsibilities

• Under the direction of the On Call RN/Triage RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC).
• Provide home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN.
• Assist the RN in caring out the POC.
• Assist the patient in learning appropriate self-care techniques.
• Provide services in accordance with organizational policies and procedures and regulations that define the scope of services.
• Assist the RN and/or physician in performing specialized duties related to end-of-life care.
• Timely submission of all documentation.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

What You Must Have
