Amorem is seeking an on-call hospice social worker to provide responsive support for patients and families in Burke and Caldwell counties. This part-time role requires timely telephone availability, collaboration with the care team, and the ability to assess psychosocial needs in alignment with the patient plan of care.

Qualifying candidates hold a Master’s or Bachelor’s in Social Work and have experience in healthcare or end-of-life settings.

Qualifications

• Ability to respond to patient/family needs with empathy and professionalism.
• Strong communication with patients, families and healthcare teams.
• Experience coordinating with an interdisciplinary hospice team to meet patient goals.

Responsibilities

• Be available by telephone and respond within 15 minutes.
• Communicate with on-call team and patient care unit staff regularly.
• Evaluate psychosocial needs and coordinate plan of care with hospice team.
• Educate patients and families about hospice services and coverage options.
