Job summary

AMOREM provides compassionate hospice care and seeks a dedicated social worker to coordinate services for patients and families in a multidisciplinary team. The role focuses on psychosocial assessment, education about hospice benefits, and facilitating access to community resources.

Responsibilities include developing care plans, supporting bereavement processes, and collaborating with chaplains and volunteers to meet patient needs in a respectful, holistic manner.

Qualifications

• Bachelor’s degree in social work (CSWE-accredited) or related field.
• Two years of clinical experience preferred.
• Experience in health care social work is advantageous.

Responsibilities

• Coordinate medical social work services for hospice patients and families.
• Evaluate physical, financial, psychological, spiritual, and pre-bereavement needs with the interdisciplinary team.
• Educate patients and families about hospice services and coverage options.
• Assist with applications and referrals for appropriate services.
• Develop care plans with goals defined by the patient/family and team.
• Provide social work services according to the plan of care.
