Qualifications

• Current RN license, state-specific
• One+ year of nursing experience required
• Current CPR certification
• Valid driver's license and reliable transportation

Responsibilities

• Performs initial nursing assessment on new admissions
• Coordinates and revises the plan of care with the care team
• Communicates patient progress to clinical manager and physician
• Maintains patient care records with timeliness and accuracy
• Educates families on patient condition and care needs
• Performs on-call responsibilities for patients and families
• Performs other duties as assigned
