Job summary

Amedisys Home Health is seeking a weeknight on-call registered nurse to assess new admissions, coordinate care plans, and support patients at home. You will collaborate with the clinical team, document progress, and educate families on care requirements.

Qualifications

Current RN license, specific to the state(s) you are assigned to work.
One+ year of nursing experience required
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Performs initial nursing assessment on new admissions.
Coordinates, revises and maintains the total plan of care with the care team.
Regularly communicates patient progress to the clinical manager, physician and care team.
Maintains individualized patient care progress records with accuracy and timeliness.
Records pain/symptom management changes/outcomes as appropriate.
Educates families on patient condition, needs and care.
Makes referrals to other hospice care disciplines as needed.
Performs on-call responsibilities and on-call services to patients/families.
