Job summary

AMEDISYS HOME HEALTH is seeking an on-call Hospice Registered Nurse to cover Monday–Thursday, 5:00pm–8:00am. You will perform admissions assessments, coordinate care plans, and communicate with the care team to ensure high-quality patient care at home.

Qualifications

• Current RN license, state-specific as needed.
• 1+ year of nursing experience required.
• Current CPR certification.
• Valid driver’s license with reliable transportation and insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the care plan and ensures continuity of patient care.
• Regularly communicates patient progress to the clinical manager and care team.
• Maintains accurate patient care records with timely updates.
• Records pain/symptom changes and outcomes; educates families about patient condition and needs.
• Makes referrals to other hospice care disciplines as needed.
• Performs on-call responsibilities and related services.
