Job summary

Generations Hospice is seeking an On-Call RN to join our team in Fond du Lac, WI. This role covers after-hours shifts and requires travel throughout the territory.

You will collaborate with an interdisciplinary team to provide compassionate, patient-centered hospice care in home and facility settings. The ideal candidate holds licensure as an RN, with 1 year of nursing experience (3+ years preferred) and a solid understanding of hospice philosophy.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps.
• If a death occurs during on-call hours, pronounce death, provide support to the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.
