Generations Hospice is seeking an On-Call RN to join our team for after-hours, compassionate end-of-life care in patients' homes and facilities.

The RN collaborates with an interdisciplinary team, handles urgent symptom management, and maintains thorough documentation. A valid RN license and hospice experience are preferred, with a 7ON 7OFF schedule and travel across the territory.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.

Responsibilities

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care based on hospice protocols or provider orders.
• Communicate with the physician regarding changes in condition and obtain orders as necessary.
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps.
• If a death occurs during on-call hours, pronounce death, provide support to the family, contact the funeral home, and notify the hospice team.
• Complete timely documentation and communication.

Schedule:Full Time 7ON 7OFF Weekdays Monday-Thursday 430p-8a & Friday starting at 4:30PM through Monday 8AM
