Job summary

Hospice Austin is seeking an RN Case Manager to provide and coordinate professional nursing care in patients' homes within the Spicewood/Austin area. The role focuses on end-of-life care, assessment, and care planning to support patients and families.

Qualified candidates will hold an Texas RN license with a BSN preferred. Previous hospice or home health experience is desirable, and strong communication with families and the care team is essential.

Qualifications

• RN license in Texas required.
• Experience in hospice or home health care preferred.
• Strong assessment and care planning skills.

Responsibilities

• Provide professional nursing care for hospice patients in the homecare setting.
• Perform initial and ongoing assessments of physical, functional, psychosocial, and environmental needs.
• Develop and maintain individualized plans of care for palliative and end-of-life needs.
• Coordinate with physicians, caregivers and team members to ensure quality care.
