Job summary

Houston Hospice is seeking an RN Field Case Manager in Houston, TX to oversee the overall management of patient cases and coordinate care with clients, families, and other providers.

You will maintain case documentation, develop care plans based on clinical assessments, and ensure timely visits and referrals to appropriate services. This is a full-time professional role focusing on collaboration with healthcare teams to improve patient outcomes.

Qualifications

• Experience coordinating care with multiple providers and families.
• Knowledge of state regulations related to case management.
• Strong communication and documentation skills.

Responsibilities

• Coordinate care with providers, including RNs and LPNs.
• Maintain case documentation in accordance with state regulations.
• Develop and maintain case plans based on clinical assessment and family needs.
• Ensure visits by primary physician or psychiatrist within 24 hours of admission.
• Coordinate care with community agencies as necessary.
• Provide training to staff on client management and referral processes.
