Job summary

Elara Caring in Oklahoma is seeking an After-Hours/On Call Registered Nurse to support hospice and home health care delivery. You will assess patients, monitor changes, and guide staff and families through the plan of care.

You must hold a current RN license, have 1+ year of clinical nursing experience, and be able to travel between patient homes. Home health or hospice experience is preferred; lifting up to 50 lbs is required; valid driver''''s license or reliable transportation is needed.

Qualifications

• RN license is required and must be current.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• At least 1 year of nursing experience in a clinical setting.
• Home health or hospice experience preferred but not required.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to physician and care team.
• Educate patients, families, caregivers, and staff in the interdisciplinary Plan of Care.
