Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse to join our hospice team on a 7-on/7-off schedule. You will assess patients and families, monitor changes, and educate care teams across homes, hospitals, and facilities. This role requires RN licensure and clinical nursing experience.

You will provide pain and symptom management education and support bereavement assessments for families, contributing to compassionate end-of-life care. Travel between patient homes is required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to lift, push, pull up to 50 pounds and travel between homes.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual and environmental areas.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the Plan of Care.
• Provide education on hospice philosophy, pain management and symptom control to staff in various care settings.
• Provide compassionate support at time of death and complete bereavement assessments for families.
