Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse to deliver compassionate, patient-centered care in home settings. You will assess needs, monitor conditions, and educate staff and families as part of an integrated care team across hospice and home health environments.

Ideal candidates hold an RN license, have 1+ year in clinical care, and can travel to patient homes. Home health or hospice experience is preferred; drivers or reliable transport are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year of experience in a clinical care setting.
• Home health or hospice experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver's license or reliable transportation.

Responsibilities

• Assess patients and families to identify needs across physical, psychological, social, spiritual, and environmental aspects.
• Monitor patient condition and report changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide hospice philosophy, pain and symptom management education to staff in care settings.
• Offer compassionate support at the time of death and complete bereavement assessments.
