Job summary

Three Oaks Hospice, Inc. is seeking an On-Call Registered Nurse to join a leading hospice care team serving North Dallas, McKinney, Plano and Rockwall.

The RN will deliver skilled nursing care at patients' homes, coordinate with the interdisciplinary team, families, and referral sources, and ensure seamless care planning. The ideal candidate will have an RN license, a current CPR certification, and at least two years of RN experience in a clinical setting, with hospice/home health experience

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification and strong computer skills.
• Excellent interpersonal skills and ability to support patients and families.

Responsibilities

• Administer skilled nursing care for clients in their residence.
• Coordinate care with the inter-disciplinary team, patient/family, and referring agency.
• Assume responsibility for coordination of care.
