Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team. The role involves providing skilled nursing care to clients in their home, coordinating with the interdisciplinary team, patient and family, and referring agencies.

We operate across a network that includes Agape Hospice Care, Sage Hospice, Elevation Hospice of Colorado, Elevation Hospice, and Primary and Palliative Care of Utah.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver's license.
• Two years of RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.

Responsibilities

• Administer skilled nursing care to clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Take responsibility for coordinating overall patient care.
