Job summary

Agape Hospice Care is seeking a compassionate On-Call Registered Nurse (RN) for weekend coverage to join Three Oaks Hospice and affiliated teams. You will provide skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and support families with clinical needs while delivering compassionate, patient-centered care.

This weekend-on-call role requires an active RN license, CPR certification, and the ability to manage care under pressure with empathy.

Qualifications

Graduate of an accredited Associate or bachelor's degree in Nursing.
Current state license as an RN and valid driver's license.
Two years' experience as an RN in a clinical care setting; hospice/home health preferred.
Current CPR certification.
Proficient with computers; Microsoft Office experience is preferred.
Strong interpersonal skills and ability to provide effective support to the patient and family.
Demonstrate skill in nursing practice and ability to cope with family emotional stress.

Responsibilities

Administer skilled nursing care for clients in their place of residence.
Coordinate care with the interdisciplinary team, patient/family and referring agency.
Coordinate care planning and provide ongoing support.
