Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our hospice care team. This role focuses on delivering skilled nursing care to patients in their homes and coordinating with the interdisciplinary team to ensure smooth, compassionate care.

Ideal candidates have an accredited nursing degree, an active RN license, CPR certification, and at least two years of RN experience in clinical settings.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice/home health preferred.
• Current CPR certification required.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patient and family.

Responsibilities

• Administer skilled nursing care in patients’ place of residence.
• Coordinate care with interdisciplinary team and referring agencies.
• Support patient and family through care planning and execution.
