Job summary

Elara Caring is seeking an On-Call Hospice RN in Carrollton, Michigan to provide compassionate hospice care in home and community settings. This Full-Time, Staff role requires travel 50% and supports patients and families after hours.

You will be a graduate RN with current state license, perform assessments, coordinate with the Interdisciplinary Team, and educate families on care plans, pain management, and comfort measures.

Qualifications

• Graduate of an accredited Nursing program (Certificate/ Diploma/ Associate/ Baccalaureate)
• Current State License as a Registered Nurse (RN)
• 1 year of clinical care experience
• Hospice or home health experience preferred
• Valid driver's license and reliable transportation

Responsibilities

• Assess hospice care patients and families to identify needs and document findings
• Communicate significant findings to Clinical Supervisor, physician, and care team
• Counsel and educate patients, families, and facilities in the Interdisciplinary Plan of Care
• Teach hospice philosophy and pain management to staff in alternate care settings
• Provide support at time of death and perform bereavement assessment
