Elara Caring in Saginaw, Michigan, is seeking an After Hours/ On Call Registered Nurse to join our hospice team. This role focuses on delivering compassionate care after hours and providing on-call support to patients at home.

As part of the hospice program, you will assess patient needs, coordinate with the interdisciplinary team, educate families, and ensure alignment with the plan of care. A current RN license, 1 year of clinical experience, and ability to travel up to 50% are required.

Qualifications

• Graduate of an accredited nursing program with RN licensure.
• Current State RN license is required.
• Minimum 1 year of clinical care experience.
• Experience in hospice or home health is preferred.
• Requires 50% travel and ability to lift up to 50 pounds.
• Valid driver's license and insurance required.

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Communicate significant findings to the clinical team and supervisor.
• Counsel and educate patients and families within the care plan.
• Provide pain and symptom management education to staff.
• Support at time of death and bereavement assessment.
