Job summary

MJHS is seeking an experienced registered nurse for after-hours on-call hospice and palliative care visits in the Union City area. You will respond to urgent patient needs, collaborate with triage staff and the on-call manager, and implement and adjust plans of care in partnership with physicians and caregivers.

The role requires nursing licensure in New York State (preferably) and the ability to support families facing end-of-life care, with CHPN certification pursued within the first year.

Qualifications

• Graduate from an accredited School of Nursing.
• Two years of experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology preferred.
• New York State RN licensure preferred; NYS driver license preferred.
• Will sit for CHPN certification within one year of hire.
• Follows NYS Standards for Professional Registered Nurse.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled after-hours.
• Coordinate with Call Center Triage and On Call Manager.
• Respond via cell phone to Triage visit requests and communicate with the on-call team.
