Job summary

MJHS is seeking an experienced registered nurse to handle on-call patient visits after hours, coordinating with triage and the on-call manager to address urgent needs. The role implements plans of care, educates patients and primary caregivers on changes, and ensures patient safety and symptom management.

Responsibilities include attending patient deaths with supportive guidance to families and reporting updates to the triage team, all while adhering to NYS Nursing standards.

Qualifications

• Graduate from an accredited School of Nursing.
• New York State RN licensure preferred.
• Will sit for CHPN within one year of hire.
• At least two years experience in Medical-Surgical Nursing, Community Health, Hospice or Oncology nursing preferred.
• Word, Excel, Windows applications.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled for urgent physical and psycho-social issues during after-hours shifts.
• Coordinate with Call Center Triage and On Call Manager to address urgent patient needs.
• Implement and educate on plan of care including medications and safety.
• Attend patient deaths to provide emotional support to family.
• Report changes to triage and Hospice team and follow up needs.
• Follow NYS Standards for Professional Registered Nurse.
