Job summary

MJHS is seeking an experienced Registered Nurse to provide on-call hospice and palliative care visits after hours, coordinating with the Call Center Triage and On Call Manager to ensure safe plans of care.

The role requires NYS RN licensure and graduation from an accredited School of Nursing; CHPN within 12 months is preferred. Medical-Surgical or Hospice/Oncology experience is valued, with proficiency in Word, Excel, and Windows applications.

Qualifications

• New York State RN licensure required.
• Graduate from accredited School of Nursing.
• CHPN certification within 12 months of hire.

Responsibilities

• Provide on-call patient visits scheduled or unscheduled for urgent physical and psychosocial issues during after-hours shifts.
• Coordinate with Call Center Triage and On Call Manager; respond via cell phone and communicate with the on-call team and hospice physician.
• Implement plan of care and educate patient and primary caregiver on changes to care, meds, safety, and symptom management.
• Attend patient deaths as directed to provide emotional support and guidance to family and report changes to triage and primary hospice team.
• Follow NYS Standards for Professional Registered Nurse and document follow-up needs.
