Job summary

The schedule is 7 On/7 Off with 5p-8a shifts Monday-Thursday and 5p-8a Friday-Monday, including after-hours calls, admissions, and ongoing assessment, care planning, and family education.

Qualifications

• Active Registered Nurse (RN) license in good standing.
• Experience in hospice, home health, or community health settings.
• Strong assessment and care planning abilities.

Responsibilities

• Respond to after-hours calls and visit patients as assigned.
• Perform comprehensive patient and family assessments.
• Develop and revise plans of care based on patient needs.
• Educate patients and families about care, medications, and procedures.
