Resource 1 Homecare Staffing in Maple Grove, MN seeks an On-Call Hospice Registered Nurse (RN) for nights and weekends on a 7 on/7 off schedule. The role focuses on providing compassionate, home-based hospice care to patients and families.

The RN will perform assessments, coordinate care, administer medications, educate families, and liaise with physicians and the interdisciplinary team to ensure high-quality end-of-life support.

Qualifications

• 1-2 years of recent acute care experience in an institutional setting (required).
• 1-2 years of recent home care experience (preferred).
• Valid driver’s license and an insured vehicle in good working order.
• Graduation from an accredited school of nursing.
• Understands hospice philosophy; comfortable with death and dying.

Responsibilities

• Respond to triage calls during assigned shifts and visit patients with changing conditions.
• Complete initial and ongoing assessments and update the plan of care as needed.
• Provide professional nursing care and administer medications as prescribed.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Educate patients and families on care plans and self-care techniques.
