Job summary

St. Croix Hospice seeks a Registered Nurse On-Call in Wisconsin (Neenah) to provide after-hours nursing visits within our 24/7 care model. The role covers evenings, nights, and weekends on a 7 days on/7 days off overnight schedule, delivering compassionate, autonomous patient care.

You will respond to triage calls, develop and adjust care plans, administer medications, and maintain accurate clinical documentation to support patients and families during end-of-life care.

Qualifications

• Graduate of an accredited nursing program (Associates, Bachelors, or Masters).
• 1+ years of recent acute care experience; hospice or home care preferred.
• Current RN license in state of practice and CPR certification.
• Must have reliable transportation, valid driver's license, and proof of auto insurance.

Responsibilities

• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and treatments as prescribed, with education and support to patients and caregivers.
• Maintain timely and accurate clinical documentation of care, outcomes, and communication.
