Job summary

Optimal Hospice is seeking an On-Call Registered Nurse in Colorado to provide comprehensive hospice care for patients and families. The role requires commitment to every other weekend and entails independent nursing assessments, care planning, and coordination with the interdisciplinary team.

The candidate must hold an active RN license, NLN-accredited program, CPR, and a valid driver’s license; hospice experience is preferred.

Qualifications

• RN licensure in the state of practice.
• Graduate of NLN-accredited nursing program.
• Current CPR certification.
• Current valid driver’s license.
• Hospice experience preferred.
• Ability to organize and prioritize workload independently.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care using all elements of the nursing process.
• Develops a plan of care with patient and family participation and revises it as needed.
• Administers medications and treatments as prescribed and provides health care instructions.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Maintains accurate clinical notes and reports changes in patient condition to the physician.
