Optimal Hospice is seeking a 7on/7off Primary On-Call Registered Nurse to provide comprehensive hospice care for patients in the Denver area. The position follows every other week Monday–Sunday 6:30 PM–8:00 AM with full-time status.

The RN must hold an active nursing license in the state, have NLN-accredited nursing education, and maintain CPR certification. One year of hospice experience preferred; driver’s license required. Competitive benefits and ongoing education are offered.

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Develops and implements care plans with goals based on nursing diagnosis.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Teaches patient and family self-care and health counseling; coordinates care with interdisciplinary team.
• Acts as Case Manager for assigned patient caseload and documents care accurately.

Responsabilidades
