Job summary

Care Plus Hospice Care LLC in the Phoenix AZ valley seeks an After Hours Hospice Nurse (On-Call) to provide expert clinical care and compassionate support to patients and families during their most critical moments after hours.

You will respond to after-hours calls, perform skilled nursing assessments, manage pain and symptoms, document in EMR, and coordinate with the on-call physician and interdisciplinary team. Evening, weekend, and holiday shifts are required.

Qualifications

Active Arizona RN or LPN license; RN preferred.
Minimum 1–2 years hospice, palliative care, or home health nursing.
Ability to work on-call evenings, weekends, and holidays.
Valid driver’s license and reliable transportation.
Proficient EMR documentation and clinical note taking.
Compassionate, calm, professional demeanor with patients and families.

Responsibilities

Respond to after-hours calls, triage and provide in-person visits as needed.
Conduct skilled nursing assessments for hospice patients at home or in facilities.
Manage pain and symptom control per the care plan.
Communicate with on-call physician and team about status changes.
Provide emotional support and education to patients and families.
Document all encounters, interventions, and updates in a timely manner.
Coordinate after-hours care including medication changes and equipment.
