Job summary

The Pennant Group, Inc. is seeking an On Call Nurse for Agape Hospice in Arizona. This role involves providing after-hours nursing care, patient visits, wound care, and coordination with the RN Case Manager to ensure seamless hospice services.

You will educate families, document care, and participate in interdisciplinary team activities while adhering to state and Joint Commission requirements.

Qualifications

• Must hold current Arizona nursing license.
• Maintains CPR certification.
• Must have valid driver's license and own vehicle.
• Minimum 2 years' experience in public health, home health, or hospice nursing preferred.
• Self-directed and able to work with minimal supervision.
• May supervise hospice aides; supervisory experience a plus.

Responsibilities

• Work outside normal hours and holidays as needed.
• Take calls from main line or answering service after hours.
• Conduct patient visits and collect information for RN Case Manager.
• Perform treatments and wound care as directed.
• Update plan of care with Interdisciplinary team; attend meetings as needed.
• Educate patient and family on medications and care.
• Assist in discharge planning and coordination of care after hours.
• Document care provided and maintain records per policy.
• Participate in quality assessment and performance improvement activities.
