Job summary

Care Plus Hospice Care LLC in the Phoenix AZ valley seeks an After Hours Hospice Nurse (On-Call) to provide expert clinical care and compassionate support to patients and families during their most critical moments after hours.

You will respond to after-hours calls, perform skilled nursing assessments, manage pain and symptoms, document in EMR, and coordinate with the on-call physician and interdisciplinary team. Evening, weekend, and holiday shifts are required.

Qualifications

• Active Arizona RN or LPN license; RN preferred.
• Minimum 1–2 years hospice, palliative care, or home health nursing.
• Ability to work on-call evenings, weekends, and holidays.
• Valid driver’s license and reliable transportation.
• Proficient EMR documentation and clinical note taking.
• Compassionate, calm, professional demeanor with patients and families.

Responsibilities

• Respond to after-hours calls, triage and provide in-person visits as needed.
• Conduct skilled nursing assessments for hospice patients at home or in facilities.
• Manage pain and symptom control per the care plan.
• Communicate with on-call physician and team about status changes.

Responsibilities:

• Respond to after-hours patient and family calls, providing triage, guidance, and in‑person visits as needed
• Conduct skilled nursing assessments and interventions for hospice patients in home or facility settings
• Manage and adjust pain and symptom control measures in accordance with the patient’s care plan
• Communicate effectively with the on‑call physician and interdisciplinary team regarding patient status changes
• Provide emotional support and education to patients and families during end‑of‑life transitions
• Document all clinical encounters, interventions, and patient updates accurately and in a timely manner
• Coordinate after‑hours care needs including medication changes, equipment, and emergency support

Requirements:

• Active RN or LPN license in the state of Arizona (RN preferred)
• Minimum 1‑2 years of hospice, palliative care, or home health nursing experience
• Strong clinical assessment and critical thinking skills in autonomous settings
• Ability to work on‑call shifts including evenings, weekends, and holidays
• Valid driver’s license and reliable transportation for patient home visits
• Proficiency in electronic medical records (EMR) documentation
• Compassionate, calm, and professional demeanor when working with patients and grieving families
