Job summary

Acacia Health, a compassionate hospice and home health caregiver, seeks an on-call LVN to provide specialized hospice visits. You will document care accurately and educate patients and families, with opportunities for daytime visits based on availability.

Applicants must hold an active CA LVN license, a valid CA driver’s license, and have hospice experience. On-call shifts and per-visit opportunities are required, with pay in the $31–$36 per hour range.

Qualifications

• Demonstrated compassion and commitment to patient-centered care.
• Active LVN license in the state of California.
• Valid California driver’s license and reliable transportation.
• Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end-of-life care.
• Prior experience in nursing or related medical fields.
• Experience as an on-call LVN runner preferred.

Responsibilities

• On-call hospice visits after hours with daytime visits.
• Provide accurate and timely documentation.
• Assess and provide patient and family/caregiver education.
• Perform other duties as assigned by the registered nurse.
• Other visits as needed based on availability including routine and PRN visits, discharges, deaths, etc.
