Responsibilities

• Explain hospice benefits and support admission process.
• Relay information from EOB meetings to Intake/Clinical teams.
• Request and organize documents for admissions.
• Address benefits eligibility questions with Intake team.
• Collaborate with Community Liaisons for referral transitions.
• Provide direct skilled nursing care under RN oversight.
• Document care plans, meds, and symptom monitoring.
• Maintain facility binders and foster positive relationships.
• Ensure timely documentation (visit notes, referrals, EOB explanations).
• Assist with admissions, discharges, and other admin tasks.
