Job summary

Acacia Hospice in California is seeking a compassionate Licensed Vocational Nurse for per diem, PRN night shifts. You will participate in on-call visits, document accurately, and educate patients and families in end-of-life care.

The role requires an active California LVN license, reliable transportation, and hospice experience. Per diem hours offer flexible scheduling, with weekends included, and a team-focused, patient-centered approach.

Responsibilities

• On-call hospice visits on weekends
• Provide accurate and timely documentation
• Assess and provide patient and family/caregiver education
• Perform other duties as assigned by the registered nurse
• Other visits as needed, including routine and PRN, discharges, and end-of-life events

Responsibilities

• Oncall hospice visits on weekends
• Provide accurate and timely documentation
• Assess and provide patient and family/caregiver education
• Perform other duties as assigned by the registered nurse
• Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.

Qualifications

• Demonstrated compassion and commitment to patient centered care
• Active LVN license in the state of California
• Valid California driver’s license and reliable transportation
• Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care
• Prior experience in nursing or related medical fields
• Experience as an on call LVN runner preferred
