Job summary

Acacia Health (Palliative, Home Health, & Hospice) seeks an on-call LVN for weekend hospice visits in California’s Inland Empire. You’ll document accurately and promptly, educate patients and families, and support the RN with care planning.

This per diem/PRN role requires an active California LVN license, reliable transportation, and hospice experience. Weekend shifts run Friday evening to Sunday morning, with visits and discharges as needed.

Qualifications

• Demonstrated compassion and commitment to patient centered care.
• Active LVN license in the state of California.
• Valid California driver’s license and reliable transportation.
• Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care.
• Prior experience in nursing or related medical fields.
• Experience as an on call LVN runner preferred.
