Job summary

AccentCare is hiring a Nurse LVN/LPN for Continuous Care in Hospice. This full-time, on-site role covers multiple counties with a schedule of Monday to Friday, 8-hour shifts. Salary ranges from $25.00 to $33.00 per hour, based on experience.

We seek an LPN with hospice, home health, or geriatrics experience, CPR certification, and a valid driver’s license. You’ll work within an interdisciplinary team, using EMR systems, and provide compassionate end-of-life care.

Qualifications

• Must hold current LPN license in practicing state
• CPR certification required for direct patient care in Georgia and preferred elsewhere
• At least one year of nursing experience in hospice, home health, or a specific patient population (geriatrics, pediatrics, med-surg)
• Must have a valid driver’s license, reliable vehicle, and meet minimum state-required auto insurance
• Able to work as part of an interdisciplinary team with knowledge of relevant state, agency, and federal hospice policies and protocols
• Physical demands include lifting up to 50 lbs and frequent patient assistance

Responsibilities

• Deliver skilled nursing care to patients and families per the plan of care
• Coordinate with care team and physicians to ensure high-quality continuous care and keep records updated
• Educate and support caregivers on medication management and personal care
• Serve as a key communicator to the Director of Clinical Operations and IDG for complex issues
• Provide compassionate support during critical moments, including at the time of death, with 24/7 coverage as needed
• Participate in teams meetings, training programs, compliance efforts and licensure-related duties
