Job summary

Suncrest Hospice in California seeks an On-Call LVN to deliver comprehensive hospice care to patients in their homes, facilities, or hospitals. You will work after hours on a seven-days-on, seven-nights-off schedule from 5 PM to 8:30 AM, traveling across the service area with a supportive care team.

Must hold an active California LVN license with at least one year of clinical experience. Strong teamwork, communication, and time-management skills are essential to succeed in this role.

Qualifications

• Requires current LVN licensure in good standing in CA.
• Minimum of one year of licensed clinical experience.
• On-call experience is preferred.

Responsibilities

• Provide care and services to hospice patients in their residences.
• Visit patients at private homes, assisted living facilities, or hospitals.
• Participate as part of a patient care team and document care provided.
