Responsibilities

• Deliver patient-centered care in a hospice setting, focusing on comfort and symptom management.
• Provide after-hours support with ongoing patient assessments and escalation as needed.
• Educate patients and caregivers on end-of-life care plans and symptom relief strategies.
• Ensure thorough EMR documentation of visits and care plans.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within the assigned territory.
