Job summary

Ohio Living is seeking an On-Call/After Hours LPN to support hospice and home health patients outside normal business hours. The role emphasizes efficient triage, timely visits, and collaboration with the interdisciplinary team to meet each patient’s plan of care.

The ideal candidate holds an Ohio LPN license, has 3 years of acute care or geriatric nursing experience, and is proficient with EMR and MS Office. Weekend/evening shifts and driving requirements apply.

Qualifications

• Current unencumbered license for the state of Ohio to practice as an LPN.
• Three years of experience in acute care, medical-surgical, oncology, or geriatric nursing.
• Experience in a Medicare-certified home health and hospice agency preferred.

Responsibilities

• Triages calls from patients and caregivers and identifies needs.
• Makes timely home or facility visits to observe changes and report to the care team.
• Communicates changes in patient condition to care providers and families.
• Assists with pain management, symptom control, and patient-family education.
• Administers and evaluates medications within the LPN scope of practice.
• Documents patient care and treatment modalities in records per standards.
• Maintains HIPAA compliance and regulatory knowledge.
