Job summary

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.
• Active RN license, CPR certification (in-person), valid driver’s license and reliable transportation.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments and routine visits.
• Assess physical, emotional, psychosocial, and spiritual needs for patients and families.
• Support the Interdisciplinary Group (IDG) with chart reviews, plan updates, and communication.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression, symptom management, and hospice philosophy.
• Maintain accurate clinical documentation including assessments and care plans.
