Job summary

Heart to Heart Hospice in Tennessee is seeking a Registered Nurse On Call to provide in-home patient assessments, treatment, and supportive care. Duties include crisis management, pronouncement of death when needed, and admissions.

The role requires an accredited nursing degree, active RN license, valid driver's license and reliable transportation, and ideally two years of RN experience in health care or clinical settings.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment and support.
• Develop and revise nursing care plans with the patient’s physician and IDG.
• Utilize excellent customer care skills with internal and external customers, including proper phone etiquette.
• Prepare clinical and progress notes that demonstrate progress toward established goals.
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
