Job summary

Blanchard Valley Health System is seeking a Registered Nurse for Home Health Care to perform ongoing patient assessment, care planning, and timely documentation of progress in the home setting. You will be responsible for on-call management as scheduled, ensuring safe, patient-centered care across the service area.

Ideal candidates hold an RN license in Ohio, have 1–2 years of general nursing experience, and possess a valid Ohio driver's license with reliable transportation.

Qualifications

• Current state licensure as a Registered Nurse (RN).
• Minimum of 1-2 years of general nursing experience.
• Current Ohio Driver’s license with clean driving record and ability to provide own transportation while on duty required.
• Commitment to and knowledge of the philosophy, mission and goals of the agency and standards and regulations governing it.

Responsibilities

• Assume ongoing assessment and care planning for home health patients.
• Document progress and ensure accountability for assigned patients.
• Manage on-call as scheduled in a consistently responsible manner.

Skills
