Zusammenfassung

Pacific Clinics seeks compassionate team members to operate within a 24/7 crisis response and stabilization program serving adults in crisis. You will provide direct crisis services, link clients to community resources, and help connect to permanent supports while collaborating with families and natural supports.

Responsibilities include crisis intervention, documentation to Medi-Cal standards, and developing strength-based plans to improve outcomes.

Qualifikationen

• Requires High School Diploma or GED; 2 years of relevant experience equals one year of education.
• Must be 18+ years old.
• Must know or quickly acquire extensive knowledge of local community resources.
• Ability to articulate the experience and perspective of individuals with complex needs involving mental health, homelessness, or probation services.

Aufgaben

• Provide direct services to individuals in crisis in community settings.
• Document in Medi-Cal compliant manner and meet productivity expectations.
• Engage with individuals' family and natural supports to align and reinforce goals.
• Use strengths-based planning to implement crisis interventions and safety plans.
• Develop hypotheses of function and unmet needs to guide individualized strategies.
• Drive to crisis locations as needed and support transitions to lower levels of care.
