Job summary

Innovate Rehab and Wellness is seeking a Licensed PRN Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility. The role emphasizes one-on-one patient care, with realistic scheduling to support work–life balance and a competitive hourly rate based on experience.

You will participate in care planning, document patient progress, and collaborate with nursing and therapy teams to deliver patient-centered rehab services under OT supervision.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.

Responsibilities

• Support delivery of high-quality OT services.
• Help residents regain independence and improve quality of life.
• Provide therapeutic interventions under OT supervision.
• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
