Job summary

Reliant Rehabilitation is seeking an Occupational Therapist to provide services in Assisted Living, Independent Living, and Memory Care settings. The role supports patient care under the supervision of a licensed OT, with responsibilities including patient evaluation input, treatment implementation, and case management within Reliant Rehabilitation guidelines.

You will collaborate with rehab teams to ensure quality care, maintain documentation, and adhere to all regulatory requirements.

Qualifications

• Graduate of an ACOTE accredited school of occupational therapy.
• Strong problem-solving and communication skills are essential.
• Ability to collaborate with interdisciplinary teams and patients.

Responsibilities

• Under the direction of the licensed OT, optimize patients' functional abilities in communities.
• Assist in screening and providing input to initial evaluation of patients.
• Implement treatment for assigned patients.
• Maintain compliant billing and documentation per guidelines.
• Supervise rehab techs during group/individual treatment.
• Contribute to building the OT assistant program and community care activities.
• Maintain relationships with healthcare professionals and community.
• Make appropriate referrals to rehab team members.
• Monitor patient progress and report issues to the OT.
• Follow infection control, safety, and privacy policies.
• Travel between communities as scheduled; work varying hours, including weekends/holidays.
