Job summary

Reliant Rehabilitation seeks an Occupational Therapist to provide outstanding OT services in Assisted Living, Independent Living, and Memory Care communities. You will screen, evaluate, and develop treatment plans, discharge plans, and supervise COTAs and rehab techs, ensuring accurate documentation and regulatory compliance.

The role requires travel between communities, strong professional conduct, and adherence to state/federal guidelines, OBRA, and Reliant policies.

Qualifications

• Competent clinical problem-solving to identify and prioritize issues.
• Strong teamwork and collaboration with interdisciplinary teams.
• Excellent verbal and written communication skills.
• Ability to build relationships with diverse populations.
• Proficiency in English for instructions, memos, and reports.

Responsibilities

• Screen, evaluate, and treat residents as referred by the care team.
• Develop initial and ongoing treatment plans and discharge plans.
• Supervise assistants and rehab techs; manage case load and documentation.
• Ensure compliance with state/federal regulations and OBRA guidelines.
• Coordinate referrals to COTAs and provide clinical supervision.
