Job summary

Reliant Rehabilitation seeks an Occupational Therapist to provide therapy in assisted living, independent living, and memory care settings. You will work under the direction of a licensed OT to deliver high-quality patient care and collaborate with the rehab team.

Responsibilities include screening, charting, implementing treatments, supervising rehab techs, and contributing to case management while adhering to state and federal guidelines. Travel between communities is required.

Qualifications

Responsibilities

• Under the licensed OT, optimize patients' functional abilities in community settings.
• Screen and provide input to initial evaluations of patients as appropriate.
• Implement treatment plans and ensure compliance with billing/d documentation guidelines.
• Provide direct supervision to rehab technicians during group and individual sessions.
• Build and sustain relationships with patients and the interdisciplinary care team.
• Travel between communities during scheduled hours and cover varying shifts as needed.
