Job summary

PACS Group in Santa Cruz is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our therapy team at Santa Cruz Post Acute. We provide resident-centered care in a supportive skilled nursing environment with flexible Full-Time, Part-Time, and PRN schedules.

The COTA will work under supervision of an Occupational Therapist to enhance residents’ functional abilities and independence, documenting progress and coordinating care with the broader team.

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) license in the state of California.
• Graduate of an accredited COTA program.
• Strong communication, teamwork, and documentation skills.
• Prior SNF experience preferred but new grads are welcome to apply!

Responsibilities

• Provide treatment interventions as outlined in the resident’s individualized OT plan of care.
• Assist in implementing therapeutic programs to improve ADLs, mobility, strength, coordination, and cognitive function.
• Document daily treatment notes, progress updates, and resident responses according to facility and regulatory standards.
• Collaborate with the occupational therapist, therapy staff, nursing team, and other interdisciplinary members to support resident goals.
• Educate residents, families, and caregivers on therapeutic exercises, safety techniques, and functional strategies.
